DLC Veterans Service Program

DLC in conjunction with Warrior Homes of Collier (WHC) has developed a DLC Veterans Services Program with an overarching goal of building meaningful partnerships with veterans in order to help them function better at home, in the community, on the job, in educational settings, and throughout life. The Veterans Case Manager is a collaborative supporter and member of the DLC treatment team charged with helping veterans identify their goals related to medical, substance use recovery, financial, mental health, housing, legal, educational/ vocational, transportation, and family needs while developing strategies to achieve their goals. The case manager will assist veterans with wellness, self-sufficiency and assist with financial independence through referrals to appropriate service

To qualify for services, veteran must have a mental health and/or substance use disorder diagnosis and has an Honorable, Less than Honorable and General discharge.
Please fax Referral Form to the attention of Bianca Borges at 239-643-7278.
